Optional Checklist to Apply for the Filipino World War II Veterans Parole Program (FWVP) Program

Before submitting your Form I-131, you must complete the following steps for each relative:

• Select Part 1., Item Number 6.A. and write in the receipt number for your Form I-130. Enter complete information about yourself (the petitioner or self-petitioner) in Part 2., including your address.
• If you are filing for someone else, enter complete information about your relative (the beneficiary) in Part 2., including their address.  If you are not filing for someone else, leave blank Part 2, questions 16-27.
• Do not complete Part 5., Part 6., and Part 7., which do not apply to the FWVP Program.
• Fully complete Part 8.
• Do not complete Part 9.
• Sign and date the form in Part 10. and include your daytime telephone number.
• If applicable, ensure the person who interpreted the form and instructions for you has completed and signed Part 11.
• If applicable, ensure the person who helped you prepare your form has completed and signed Part 12.

Include all requested documentation, including any documentation of recognized military service and evidence of a qualifying relationship with the veteran that existed on or before May 9, 2016. If you cannot establish military service, we will review any of your prior military service records that are available.

If available, include a photocopy of your Form I-130 approval notice (Form I-797, Notice of Action), or a printout from your USCIS online account showing the approval of your Form I-130. If your Form I-130 is pending, you may call the USCIS Contact Center at 800-375-5283 (TTY: 800-767-1833) to request expedited processing of your Form I-130. If the Form I-130 petitioner has died, include evidence that USCIS reinstated your Form I-130. If it hasn’t been reinstated, include either a request for humanitarian reinstatement or an explanation that your request for humanitarian reinstatement is currently pending with USCIS.

Include the fees for each Form I-131 you’re filing. If you’re eligible for a fee waiver, also submit Form I-912, Request for Fee Waiver, with supporting documentation.

Submit Form I-134, Declaration of Financial Support, for each relative and include the requested documentation, such as financial statements that demonstrate you will support your relative financially while they’re in the United States. Include Form I-134 for each additional financial sponsor (if needed).

If an attorney or accredited representative helped you prepare your Form I-131, submit a completed Form G-28, Notice of Appearance as Attorney or Accredited Representative, along with your Form I-131.
